Improving health, wellbeing and resilience

Connect for Health is a social prescribing service delivered across Suffolk*

*Woodbridge, Melton, Wickham Market, Alderton

The Connect for Health service is funded by Ipswich and East Suffolk CCG Transformation Fund and will work across 6 GP surgeries within the Woodbridge community. The service is designed to help individuals who are over 18 years old with a variety of non-medical challenges. Through signposting and support, the Connect for Health service aims to improve health and wellbeing and reduce social isolation.

What is social prescribing and how does it work?

Social prescribing aims to improve individuals’ wellbeing by recognising that they are affected by a range of non-medical social, economic and environmental factors. Support for many of these challenges can be often provided by local Voluntary, Community or Social Enterprise (VCSE) groups, if individuals knew how to access them.
 
Individuals connected into the Connect for Health service (either by self-referral, through health or social care providers or other local public and voluntary sector organisations) will meet with a trained Community Connector who will work with them to identify what is important to them.

Together they will develop a personalised action plan. The Community Connector will then signpost them to local sources of support, identifying their interests and priorities and finding ways of using their strengths.
 
Our Connect for Health team will work with Suffolk to improve the lives of those who live there.

  • Improved health, wellbeing and resilience.

  • Improved ability to manage long term condition(s).

  • Reduced social isolation and loneliness.

  • Increased level of confidence, self-esteem and feelings of control about decisions.

  • Improved access to services.

  • Increased participation in employment, education or training.
